New Patients Registration
It is our policy that all new patients registering at the Centre undergo a medical check-up which forms part of the registration procedure. Registration cannot be completed until this check has been carried out.
Please note that during the registration appointment, you will be asked for your ethnicity.
Furthermore proof of identification is required for all foreign nationals.
You can join the Centre if you live inthe following areas of Basildon :
- Langdon Hills
- Laindon
- Lee Chapel North
- Lee Chapel South
- Kingswood
- Ghyllgrove
Regrettably, if you move outside the above areas it will be necessary for you to register with another GP.

If you wish to register, click on the links below to open the forms. The forms are in PDF format so you must print them and fill in all the details. Once you have done this, bring the forms into the surgery so that registration can take place. Please do not post. We can then give you a new patient medical appointment.
